The Awards of Distinction recognize chapters with outstanding programs in health education, outreach, communications and collaboration.

Requirements

  • Submissions must include a narrative abstract and supporting program documents.
    Please prepare these documents before making your submission to ensure a complete nomination package.

Procedures and Guidelines

Nominations will be accepted from NHF chapters, volunteers, community members and HTC staff. Former and current NHF staff are ineligible to submit nominations.

Any individual who meets the requirements is eligible to receive an award. Current NHF board members, NHF staff and paid consultants are ineligible to receive an award.

Deadline

Deadline for submissions is Tuesday, March 15, 2016, at 11:59PM (ET).

Categories & Criteria

  • Communications: This award recognizes outstanding communications that successfully execute a defined strategy addressing matters pertinent to the bleeding disorders community.
    • Demonstrates creative approach for reaching defined audience
    • Meets the organization’s stated objectives for the program
    • Effectively uses defined medium(s)
    • Communication mediums must include at least one of the following: web, printed materials, social media, email, broadcast media, print media, online media.
       
  • Health Education: This award recognizes effective, creative programs in health education and prevention targeting subgroups such as women, parents, families, teens, adults or young adults in the bleeding disorders community.
    • Demonstrates improvement in health knowledge and/or outcomes
    • Meets the organization’s stated objectives for the program
    • Models collaboration with the local area HTC as well as other community-based organizations
    • Is creative & innovative
    • Is sustainable
    • Serves as a useful program model for other chapters
       
  • Outreach: This award recognizes effective, creative programs that identify and reach out to unserved or underserved people with bleeding disorders, including but not limited to: model programs for racially/ethnically diverse populations and outreach to women with bleeding disorders.
    • Demonstrates improvement in health knowledge and/or outcomes
    • Meets the organization’s stated objectives for the program
    • Models collaboration with the local area HTC as well as other community-based organizations
    • Is creative & innovative
    • Is sustainable
    • Serves as a useful program model for other chapters
       
  • Tami Wood-Lively Collaboration Award: This award recognizes educational programs demonstrating outstanding efforts in collaboration between HTCs and chapters, successfully affecting the broader community because of their collaboration. Tami Wood-Lively’s vivacious personality and lust for life were contagious, and she was a staunch advocate for the bleeding disorders community.  She was a brilliant, passionate and dedicated woman who believed in both the excellent model of care of the HTC, and the benefits of education and support that chapters can offer to those with bleeding disorders.  Tami served as the Regional Coordinator for Region V East with the Hemophilia Foundation of Michigan and then as the Director of Affiliate Services for the American Thrombosis and Hemostasis Network.
    • Clear evidence of the program planning process between the HTC and chapter
    • Displays collaboration between the HTC and chapter in educating the bleeding disorders community
    • Is a sustainable partnership
    • Is creative and innovative
    • Serves as a useful program for other HTCs and chapters
